When you submit a consultation request, intake form, or contact us, we may collect: your name, email address, phone number, mailing address, professional license type and number, NPI number, Tax ID/EIN, malpractice insurance details, educational history, employment history, DEA registration (if applicable), professional references, and any information you voluntarily share about your practice.
Information collected automatically:
When you visit our Site, we may collect: IP address, browser type, device type, operating system, pages viewed, referring URLs, and date/time of access through standard server logs and analytics tools.
Information from third parties:
We may receive information from CAQH ProView, NPPES, insurance payers, and licensing boards as part of the credentialing process conducted on your behalf and with your authorization.
We use your information to: process credentialing applications with insurance payers on your behalf, submit and manage insurance claims, set up and manage your Availity and payer portal accounts, provide SimplePractice and EHR training, manage your CAQH profile and re-attestations, respond to your consultation requests and inquiries, communicate with you about the status of your credentialing and claims via email, phone, or SMS (with your consent), maintain and improve our services, comply with legal and regulatory requirements, and send you service-related communications.
As a business associate of healthcare providers, we comply with the Health Insurance Portability and Accountability Act (HIPAA) and its implementing regulations. We maintain administrative, technical, and physical safeguards to protect Protected Health Information (PHI). We execute a Business Associate Agreement (BAA) with each client before accessing or handling any PHI. All documents are stored in HIPAA-compliant cloud storage. Consultations are conducted via HIPAA-compliant Zoom. Claims are submitted through secure clearinghouse connections via Availity. We do not use or disclose PHI except as permitted or required by the BAA, HIPAA, or applicable law.
By providing your mobile phone number and opting in to receive SMS communications from Foundational Provider Credentialing, you consent to receive text messages related to your credentialing services, including application status updates, claims notifications, appointment reminders, and important service alerts.
Opt-In: You must explicitly opt in to receive SMS messages from us. Consent is not a condition of purchase or service. You may opt in through our website SMS consent form, during your onboarding process, or by texting START to our business number.
Message Frequency: Message frequency varies based on your credentialing activity. You may receive up to 10 messages per month during active credentialing, and up to 4 messages per month during the Growth & Operations phase. Messages are sent only during business hours (Mon–Fri, 8 AM – 6 PM CT) unless you request otherwise.
Message Content: Messages may include: credentialing application status updates, claims submission confirmations, CAQH re-attestation reminders, appointment/consultation reminders, urgent compliance alerts (e.g., expiring licenses or malpractice), and service-related notifications.
Opt-Out: You may opt out of SMS communications at any time by texting STOP to our business number. You will receive a confirmation message and no further SMS messages will be sent. Opting out of SMS does not affect your service — we will continue to communicate via email.

Carriers: Supported carriers include but are not limited to AT&T, T-Mobile, Verizon, Sprint, and other major U.S. carriers. Carriers are not liable for delayed or undelivered messages.
No Sharing: We will never sell, rent, or share your mobile phone number or SMS opt-in consent with third parties for marketing purposes. Your phone number is used solely for the purposes described in this policy.
We share your information only as necessary to provide our services: with insurance payers (BCBSTX, UHC, Aetna, Cigna, TMHP, Superior, Molina, Driscoll) as part of credentialing applications you have authorized, with CAQH ProView for profile management, with Availity and Optum for claims submission and portal setup, with our HIPAA-compliant cloud storage and communication providers, as required by law, regulation, or legal process, and with your express written consent. We do not sell your personal information to third parties. We do not share your information for advertising or marketing purposes.
We implement industry-standard security measures including: encryption of data in transit (TLS/SSL) and at rest, access controls limiting data access to authorized personnel, HIPAA-compliant cloud storage with signed BAAs, regular security reviews, and secure disposal of records no longer needed. While we take reasonable steps to protect your information, no method of electronic transmission or storage is 100% secure.
We retain your information for as long as necessary to provide our services and comply with legal and regulatory requirements. Credentialing records are retained for a minimum of 7 years from the date of last service, consistent with industry standards. You may request deletion of your personal information by contacting us at [email protected], subject to our legal retention obligations.
You have the right to: access the personal information we hold about you, request correction of inaccurate information, request deletion of your information (subject to legal retention requirements), opt out of SMS communications at any time by texting STOP, and withdraw consent for non-essential communications.
