By accessing or using the website at foundationalprovider.com ("Site") or engaging the services of Foundational Provider Credentialing ("Company," "we," "us," or "our"), you agree to be bound by these Terms of Use. If you do not agree, please do not use our Site or services.
Foundational Provider Credentialing provides insurance credentialing, claims submission, Availity and payer portal setup, EHR training, CAQH management, group NPI setup, and ongoing Growth & Operations support exclusively for licensed behavioral health professionals in the state of Texas. Our services include credentialing with up to 8 insurance payers (BCBSTX, UnitedHealthcare, Aetna, Cigna, TMHP, Superior Health Plan, Molina Healthcare, and Driscoll Health Plan), claims submission through Availity and Optum, and SimplePractice / EHR billing configuration.
Our credentialing services are available only to fully licensed behavioral health professionals in the state of Texas, including: Licensed Professional Counselors (LPCs), Licensed Clinical Social Workers (LCSWs), Psychologists (PhD/PsyD), Psychiatrists (MD/DO), Psychiatric Mental Health Nurse Practitioners (PMHNPs), Physicians (MD/DO), and Physician Assistants (PAs). LPC-Associates, LMSWs, and other associate-level or pre-licensed providers cannot be independently credentialed with insurance payers. However, associate-level providers may be added to a group practice roster under the supervision of a qualified supervisor (e.g., LPC-S) through our Starter, Growth, or Pro plans.
As a client, you are responsible for: providing accurate, complete, and truthful information on all intake forms and documents, maintaining a valid and active Texas professional license throughout the service period, maintaining active malpractice insurance coverage, responding to our requests for additional information or documentation in a timely manner, notifying us promptly of any changes to your license status, address, practice information, or insurance coverage, paying all fees as described in your service agreement, and complying with all applicable laws and regulations related to your professional practice. Failure to provide accurate information or maintain required credentials may result in delays, denials, or termination of your credentialing applications.
Insurance credentialing is an extensive and time-consuming process. Typical payer processing times are 60–120 days for commercial payers and 90–180 days for TMHP (Texas Medicaid). These timelines are controlled by the insurance payers and are outside of our control. We submit all applications as quickly as possible and follow up bi-weekly, but we cannot guarantee specific approval dates. Clients must exercise patience during the credentialing process. We provide regular status updates through our client portal. Delays caused by incomplete documentation, payer backlogs, or factors beyond our control do not constitute a breach of our service agreement.

Monthly fees are billed on the subscription date each month during the 6-month Active Credentialing phase. After the credentialing phase, clients transition to Growth & Operations pricing. Claims beyond the monthly included amount are billed at $2.40 per claim submission. Unused claims do not roll over. Fees are subject to change with 30 days written notice.
All fees are non-refundable once the credentialing process has begun. If a payer denies your application for reasons within our control, we will resubmit at no additional charge.
You own your credentials at all times. All credentialing is performed under your individual NPI and Tax ID (or your group's NPI/Tax ID). All payer contracts are between you and the insurance company. If you discontinue our services, your NPI, CAQH profile, payer contracts, and all credentials remain yours permanently. We do not take ownership of, restrict access to, or place liens on any provider credentials.
To the fullest extent permitted by law, Foundational Provider Credentialing shall not be liable for: payer decisions to deny, delay, or terminate credentialing applications, lost revenue or business opportunities resulting from payer processing delays, errors in information provided by the client, changes in payer policies, enrollment windows, or fee schedules, technical issues with third-party platforms (Availity, Optum, CAQH, SimplePractice), or any indirect, incidental, consequential, or punitive damages. Our total liability for any claim arising from our services shall not exceed the total fees paid by the client in the 6 months preceding the claim.
We maintain strict confidentiality of all client information. We execute a Business Associate Agreement (BAA) with each client as required by HIPAA. See our Privacy Policy for complete details on data collection, use, storage, and security practices.
If you opt in to receive SMS communications, you agree to the SMS terms described in our Privacy Policy, including message frequency, content, and opt-out procedures. Consent to receive SMS is not required to use our services. You may opt out at any time by texting STOP.
Either party may terminate the service relationship with 30 days written notice. Upon termination: all credentialing applications in progress will be transferred to the client's control, all credentials, NPI numbers, CAQH profiles, and payer contracts remain the client's property, outstanding fees through the notice period remain due, We reserve the right to terminate services immediately if a client provides false information, engages in fraudulent activity, or violates these Terms.
All content on this Site — including text, graphics, logos, images, and software — is the property of Foundational Provider Credentialing and is protected by copyright and trademark laws. You may not reproduce, distribute, or create derivative works from any content on this Site without our express written permission.
These Terms of Use are governed by and construed in accordance with the laws of the State of Texas. Any disputes arising from these Terms or our services shall be resolved in the courts of Hidalgo County, Texas.
We may update these Terms from time to time. Changes will be posted on this page with an updated date. Continued use of our Site or services after changes constitutes acceptance of the revised Terms.
